[Labor Accident Case] Poisoning during the process of washing crystals using acetonitrile.

A case of poisoning that occurred during the process of washing crystals in a centrifuge using acetonitrile.

After transferring the crystals generated in the reaction vessel to a centrifuge and filtering them, we washed them with acetonitrile. However, due to insufficient washing, we switched to a kneading wash method that involved inserting hands into the centrifuge. We removed the duct of the local exhaust system of the centrifuge, which was obstructing the work, and while performing the task, we inhaled acetonitrile vapors, leading to exposure. The next day, four workers engaged in the kneading wash of the crystals reported feeling weak and went to the hospital for examination, where they were diagnosed with acetonitrile poisoning. [Suggestions] - Workers should wear personal PID monitors while working. - This will allow for monitoring exposure conditions and assist in the risk assessment of chemicals. *For more details, please refer to the PDF document or feel free to contact us.*

[Labor Accident Case] Carbon monoxide poisoning caused by coffee beans.

Introducing a case that occurred inside the container used to extract coffee concentrate installed in the canned coffee manufacturing line.

During the extraction of coffee concentrate by pouring hot water into an extraction container filled with roasted coffee beans, carbon monoxide that had been adsorbed onto the roasted coffee beans was suddenly released and accumulated in the sealed extraction container. A stick used to level the coffee beans that had fallen into the container was attempted to be retrieved after extraction, resulting in an accident. The victim was affected while discharging the remaining coffee grounds and fell inside the container. By the time they were rescued from the extraction container, they were already unconscious, and a subsequent hospital diagnosis revealed carbon monoxide poisoning, leading to their death about a month later. **Suggestion** - To prevent carbon monoxide poisoning during work, such as leaks from the extraction container, the use of portable carbon monoxide monitors is recommended. *For more details, please refer to the PDF document or feel free to contact us.*

[Labor Accident Case] Organic Solvent Poisoning in a Metal Product Cleaning Device

Introducing a case of organic solvent poisoning during non-regular operations in a metal product cleaning device.

While a basket was rotating in the washing tank of the cleaning device, it came loose, causing the products being cleaned to scatter inside the tank. A worker entered the cleaning device to pick up the scattered products. However, since they entered while there was still about half of the cleaning solution remaining, they inhaled the vapor of dichloromethane contained in the cleaning solution and were exposed. The worker lost consciousness after inhaling the vapor and was found in a state caught on a hanger inside the cleaning device by the cleaning operator, who rescued them. They were diagnosed with dichloromethane poisoning by a doctor and took 15 days off work. 【Suggestions】 ■ Measuring gas concentration is effective for confirming adequate ventilation. ■ Using a personal PID monitor for chemical risk assessment is recommended. *For more details, please refer to the PDF document or feel free to contact us.

[Labor Accident Case] Carbon monoxide poisoning during the cleaning of the adsorption tower of a desulfurization unit at a steelworks.

We will introduce a case of carbon monoxide poisoning that occurred during the cleaning of the adsorption tower of a desulfurization device at a steel mill.

During the cleaning of the lower section of the desulfurization unit, the victim and two colleagues were using a vacuum hose to perform suction work outside the inner manhole, while four others were inside the inner manhole. The observer at the steel mill sensed an unpleasant odor and, concerned about the inflow of exhaust gas from the sintering machine, measured the levels of sulfur dioxide, oxygen concentration, and carbon monoxide concentration inside the manhole. The oxygen concentration was 18.8% and the carbon monoxide concentration was 130 ppm, prompting the observer to instruct the workers inside the manhole to "run away." After receiving medical attention, both the observer and the victim were advised to be hospitalized; however, the observer returned home the same day, while the victim was hospitalized for a week due to carbon monoxide poisoning. [Suggestions] - Workers should wear portable gas monitors while working. - This will enable early detection of gas leaks in the furnace and alert workers and those nearby to potential dangers. *For more details, please refer to the PDF document or feel free to contact us.*

[Labor Accident Case] Poisoning during maintenance work on semiconductor manufacturing equipment.

Introducing a case of poisoning that occurred during maintenance work on semiconductor manufacturing equipment.

After a meeting regarding the baking and oxidation work conducted by the semiconductor manufacturer for maintenance, harmful substances were detected using a detection tube while measuring the residual harmful substances in the semiconductor manufacturing equipment. During this process, the harmful substances leaked, and the individual was affected while passing by the equipment. The affected person, after resting in the office for about an hour due to feeling unwell, received a consultation from the manufacturer's industrial physician. Following the industrial physician's instructions, they went to a university hospital for further examination, where they were diagnosed with acute drug poisoning and required a four-day hospitalization. 【Suggestion】 ■ It is recommended to use gas monitors to check environmental concentrations before work, confirm leakage during work, and verify restoration to original conditions after work. *For more details, please refer to the PDF document or feel free to contact us.

[Labor Accident Case] Leaked refrigerant ammonia flowed in, causing ammonia poisoning.

A case of ammonia poisoning occurred when refrigerant ammonia leaked from a refrigeration unit and flowed into a meat processing plant.

Due to a malfunction of the electromagnetic valve in the cooling system of the meat processing plant, the compressor in the refrigeration unit was damaged, causing ammonia gas to flow and spread within the factory. After ventilating and evacuating the ammonia gas, work commenced, but due to insufficient removal of the ammonia gas, secondary damage occurred in the form of ammonia poisoning. Additionally, a maintenance worker who came into contact with a low-temperature object while closing the valve of the refrigeration unit suffered frostbite on their toes. 【Suggestions】 ■ By using gas detectors during regular patrol inspections, it is effective for confirming the safety of workers and for the early detection of ammonia gas leaks. *For more details, please refer to the PDF document or feel free to contact us.
